I WON I WON...A $1,200 VISA GIFT CARD

Hi Xavier,

You were selected as a runner up winner in the Twisted Tea– #TwistedPhilly Photo Contest.

As a winner you will receive a $1,200 gift card with an Approximate Retail Value (“ARV”) – $1,200.

In order to claim the prize, we are legally required to have you fill out and return the attached affidavit and W9. The affidavit will need to be notarized. If you wish to accept please review and email your paperwork back by 6/27/22.
