I just recieved an affidavit in the mail from Rodale,Inc saying I am a potential winner in the Krafts Heart Your Heart Sweepstakes. It is just a typed form asking for name ,birthdate address,phone,ssn and signature. Does not need to be notarized and did not come with return envelope. Please help must be returned by june 8. :smt017

It does sound a little iffy what with no return envelope and no notary. What was the prize? The only thing I would be worried about is giving my ssn. Is there a phone number on the form? :smt017

Hi, Here is a link to the sweepstakes site that I think that you may have entered. There were--TWO--separate sweepstakes. One was for a cruise. The other was survey sweepstakes where the prize was a copy of the south beach diet heart program. Here is the info from SA for the latter contest

Rodale, Inc. is a legit magazine publisher. They publish Prevention and Bycycling and lots others. I won from them 2 years ago - a trip to Napa Valley, CA. They fedexed an affy to me and I had to get it notorized.

I received the same letter but the area for the SSN was x'ed out as though it is not required. I filled it out and sent it back without a SSN..

Good thinking on leaving out the SSN. They'll have your other info, so if they really need it for tax purposes they can ask again. You can always do a little Google "research" on these types of correspondence. Look up the return address, the name of the contact, telephone #, or any such item to see if you find a legit source. Cross check with Rodale, Inc. info available.
