Is this unusual???

I'm not really "new" to sweeping, but I've never won anything that required an affy. I've won a set of mini casserole dishes with an ARV of $60 and am required to fill it out to claim the prize. About a month ago I won a pair of boots (ARV $185), no affy required. Soooooo,,,,,,is this unusual? :scratch:
 
not unusual just something a lot of sponsors do, as long as it is a sweep you remember entering . Affy is an eligibility statement for the sponsors
 
I once had to fill one out for a $25 gift card. It depends on the company. Honestly, this is a good audit tool for the company. That way they have the backup documentation if they need it.

You should always claim all your wins on your taxes, but, if you don't, you should claim anything you fill out an affy for.

not so unusual, the funny one i had to fill an affy out for was the t.v. land instant awhile ago, had to fill it out for a rubic's cube lmao :laughing:
 
Sponsors are required to have you fill out a tax form (i can't remember the number) if the prize is worth over $600. In those cases they usually require an affy as well. But sponsors have the right to ask for an affy and/ or your tax information for any prize. I once filled out an affy and tax form for a t-shirt.
 
I once had to fill out an affy for a cooler I won from a local convience store. ARV $15. I also once received a coupon for a free 12-pack of soda in a registered letter I had to go to the P.O. to sign for. Thought I'd won BIG! LOL Congrats on the win!
