In order to claim this prize, you are required to complete an affidavit of eligibility (see attached to this email). The affidavit must be submitted within three (3) calendar days of notification (due on or before July 25th). Please note that you are NOT required to get your affidavit notarized just will need to attach a copy of a driver’s license, U.S. Passport or birth certificate.
You may submit your affidavit with attached copy of driver’s license, U.S. Passport or birth certificate via fax to 602-640-0502, or scan and email it back to [email protected]. If you fail to submit the affidavit and attach a copy of a driver’s license, U.S. Passport or birth certificate within three (3) calendar days of notification, we will understand that you are choosing to forfeit this prize.
