HELP! Won a prize but don't know where to send consent form!!

HI! I just recently won on the Honeycomb "Make Beeboy Go Buzz-erk" IW Game...I won 2 Print-N_Go tickets to Six Flags. Once the message came up that I won, it says to print the page so I printed the page and then got out of that screen on the computer. When I read the consent form, it says to fill out and mail or fax to the address at the bottom of the form. There was no address at the bottom! I have no idea where to send this to and there's no phone number for me to call!
Has anyone else won this and could you please send me the address???
