Last year I had a win that didn't require that the affidavit be notarized. That was a first for me as every other affidavit I've received required that it be notarized.
If you can't contact the Promoter for an answer then I'd get it Norarized as an abundance of caution. The Notary has or should have a rubber ink stamp for use on a document that doesn't have the Notary acknowledgement pre-printed on the document. Google "Notary stamp wording" to see samples of these stamps.
