Here is a partial email I got yesterday,
"Congratulations! We are pleased to announce that your entry was selected as a prize winner in the Athleta Revelation Capris. The prize includes one (1) $100 Gift Card.
For prizes valued at $550 or more, please complete and return the Affidavit of Eligibility and Liability Release, the Publicity Release, and W-9 within ten (10) days of receipt of this email. Once we receive these completed documents, we will send you the prize.
Thank you for your participation in our sweepstakes. We hope you enjoy your prize!

I'm not sure if I need to contact them or not. It looks like I don't have too.

Based on the portion of the message you posted, you don't have to contact them since your prize is under $550---

---I would still contact them. It is safer than "ignoring" the email. I would thank them and ask them to confirm that, as you understand it, your prize will be sent without further action on your part. I would also ask approximately when and how it will be sent, so that you have an idea what to watch for (and you won't worry unnecessarily).

I would send myself a copy of the message and save it, along with any response you receive.
 
Thanks for your reply. The email is a no-reply email, but like you suggested I'm going to contact them to make sure.

Oh, I see. Maybe the email is standardized, like a form letter, sent to all their winners to make it easier for them.

If you have contact information, then it certainly would be good to use it, especially to ease your mind, unless somewhere the email states that prizes under $550 will be sent to winners without any further verification. I understand that while your email doesn't sound iffy, it also may not cover the whole picture as well as you would wish...a case of better to be safe than sorry. :)
 
I won a Harry Potter DVD I had to fill out a form the DVD is worth about $12.00. Received it in about 5 days after I sent the form back. Good luck hope its worth more then my prize.
