I won 2 sweepstakes in April. One small and the other medium size. The small one was a pendant and a book with the signature of the author. The medium one it was a $1,000 shopping spree and products from the sponsoring company. My first question is regarding the medium size one. I sent my information right away by mail and also by fax because I only had five days to do that. The sweepstakes fullfilment company told me that they received them right away and they were going to do their work. I have waited patiently for a month because the rules said that it takes 45 days. I just received an e-mail from the sweepstakes fullfilment company asking me to fill the IRS form W9. The comapnies from the other 2 sweepsatkes that I won in the past that were about the same amount of money, didn't send me this form and the form is kind of weird. Can anybody tell if they have filled this form when they have won a prize?
My other question is regarding the small sweepstakes. I received a pendant and the cover of the book signed by the author and 10 sticky notes with the name of her other books signed by her too but I thought that I was supposed the receive the book. I go to the website and the information there is the current one (May) and cannot verify this. I have googled April but the information from April is gone. Any idea about how can I verify what I was supposed to receive?
