2 sweepstakes in April and 2 questions

I won 2 sweepstakes in April. One small and the other medium size. The small one was a pendant and a book with the signature of the author. The medium one it was a $1,000 shopping spree and products from the sponsoring company. My first question is regarding the medium size one. I sent my information right away by mail and also by fax because I only had five days to do that. The sweepstakes fullfilment company told me that they received them right away and they were going to do their work. I have waited patiently for a month because the rules said that it takes 45 days. I just received an e-mail from the sweepstakes fullfilment company asking me to fill the IRS form W9. The comapnies from the other 2 sweepsatkes that I won in the past that were about the same amount of money, didn't send me this form and the form is kind of weird. Can anybody tell if they have filled this form when they have won a prize?
My other question is regarding the small sweepstakes. I received a pendant and the cover of the book signed by the author and 10 sticky notes with the name of her other books signed by her too but I thought that I was supposed the receive the book. I go to the website and the information there is the current one (May) and cannot verify this. I have googled April but the information from April is gone. Any idea about how can I verify what I was supposed to receive?

Yes, I won a trailer last year and I did have to fill out a w-9 for them...I hope this helps.

I won $25,000 between two sweeps and neither of them required a drivers license, I won a movie from SheKnows and they wanted a copy of my license and a affidavit sent back. All of the sweeps are different and require different steps. Some wants an W9(filled out this for a trip I won) and some don't. Also FYI, I won a big prize that took me four months after the sweep had ended to receive the prizes. And on your questionable prize, without knowing the name of the sweep we can't even help you look for it.
 
Companies should be requesting a W-9 be completed for any prizes more than $600. Don't be surprised at year end if you have submitted the W-9 when you get a 1099 Misc form to report on your taxes. I had to report $4200 in 2011 for two big wins and had a nice tax bill to the IRS, but hey the prizes were great!
 
I had won a trip in February. They required all the regular affy stuff and a copy of my drivers license. I called them and told them I didn't feel comfortable sending my ss number and DL info. they told me to black out my number. They just needed to verify my name. I felt better after that. It was kinda weird. Someone could do alot of damage w/that info
 
Sweepstakes law not what a sponsor/JA wants to know about you determines what they HAVE to ask for/confirm.

And as always, check YOUR State law.

For example, Ca doesn't allow alcohol sweeps, even if they don't give away any alcohol :crazytongue:

Mich won't allow it's residents enter tobacco sweeps.

I'm sure there are more quirky State laws I'm forgetting! :whistle:

Thanks for all your answers. I feel so much better now. I have always reported my wins when I do the taxes but it's the first time that I receive the W9. I was a little nervous because I have to send my SS #. My small win was from Writerspace. I won the Sandra Hill April contest but I cannot retrieve the information on what I was supposed to receive.
