Signing the affidavit verifies that you are 21, you and your immediate family members do not work for Anheuser-Busch or their affiliates (wholesalers, distributers, retailers, agencies, etc.) and you have read and comply with the Official Rules. You need to fill out the affidavit completely and return it by January 19, 2019. You can return the affidavit by fax (xxxxxxxxxx), scanning and emailing back to this email address or mailing it to:
We encourage you to initially return the affidavit via fax or email to speed up the process, but we'll also need you to mail the original to the address provided above.
