:sunny: Just received a Winner's Affidavit of Eligibility from Women's Health Magazine, Win Today Sweepstakes, I have been chosen the winner of Camelbak Bottles!!! I have printed the affidavit and will fill out and return tomorrow, it does not say anything about having a notary signature, which I would be surprised, but double checking with anyone who wants to answer, do I just fill it out then and send to them? Do you send regular first class mail or do you go ahead and ask for confirmation of delivery. Thanks!!!!!

if it doesn't mention anything thing about notarized it doesn't have to be I have won prizes that didn't have to be notarized

So have I. I'd just send it back and keep a copy. If it doesn't need to be notarized, you can probably just fax or email the forms to them. I've been able to do that before. Congrats on a very nice win!

I won from them last year. I didn't bother with returning it certified and/or trackable. I didn't notarize either. I did keep a copy with my documenting the date sent. They subsequently lost my letter or misplaced it but upon my follow up took a copy via email. I did receive the item I won. Just some thoughts... GL :sunny:
