In order for us to verify your eligibility, it is necessary for you to complete and return an Affidavit of Eligibility and Release Form (Affidavit) and Request for Taxpayer Identification Number and Certification Form (W-9). Please do the following:
1 – Print this email and complete the Affidavit form found at the bottom of this email.
2 – Download and print the W9 form at https://www.irs.gov/forms-pubs/about-form-w-9 then complete the required information. If you cannot click on the link above, please copy and paste it into your web browser.
3 – Fax both completed documents to: (516) 365-5250 Attn: Alex Green no later than September 13, 2021.
 
I won this as well. I actually ended up sending a message back, and he said it's fine to just email them back the completed forms as a pdf. Much easier than faxing!

Oh AWESOME! Hubster switches to day shift on Saturday & I was wondering if/when I'd be able to even go do this!
