I still haven't received my prize. I filled out and signed the 1099 affidavit. I thought maybe they were still verifying my income so I emailed the sponsor with employer information to update. The event is less than 3 weeks away and I have to book travel through booking.com.

More than likely they will send the tickets like 3 days before the event. I won tickets from Quaker and that's what they said.
 
I was reading over the rules and it said I would receive an invitation for the app Visa Go no less than 20 days before the event. Well, it's now 7 days and I have not received the invitation and there's no credit in my Booking.com account. I filled out the declaration and saved a copy for my records and looked over it and everything looks correct. I emailed the sponsor 3 times and have not received a reply.

I would try to find out which ad agency is running the contest and contact them. I won an AMG contest and they never sent the included visa card. I tried asking over email and got no response. I went to their website and sent in a complaint and only then did they finally send it.

It's Don Jagoda, a very prominent name in the sweepstakes world so I know it's not a scam. They only gave me that one email address with a first name and never got a response. The rules clearly state the bank is not the actual sponsor and they're using a 3rd party to process the prizes. Visa is involved as well, so I may try to reach out to several.
