TAX HELP--Any suggestions??

I won a prize in 2015 where I had to complete a W9 tax form and sign an affidavit for publicity release. However here it is 2/14/16 and I still have not received a 1099-MISC for the win. I am not sure what to do, the prize was valued at well over $600 but not sure of the exact figure. I tried contacting the sponsor, not sure if the email is still active but I have sent 3 emails and left 2 vmails for the original contact but I have not received any response as of yet. Any suggestions on what to do?? I would like to get my taxes done but not having this form not sure what to do, did the IRS get a form, will I get one even though I were I should have already received and what the exact amount is...any suggestions greatly appreciated.
 
Not giving advice, just stating what I did this year, for taxes. I did my taxes without forms, I had to put my income as "other income", but wasn't prize income. But I would claim a prize as "other income ", if I didn't have the form. I had no w-2, for the income I claimed, there's no w-2 for the kind of income I claimed. I had to pay to have my taxes done because I sold my house. But I did my own taxes, using TurboTax. And I'm covered too by their guarantee. Previous years, I did not pay a penny. Not trying to endorse them, just stating the company I used since I don't know what other tax company would do. Last of all, I got my tax refund! Without any forms, except a 1098 form for mortgage interest from my house.
 
Let me add: when doing my taxes, you are asked if you have other income to claim, then you state what kind of income. That's where I would put it. I hope that makes sense!
 
You should always receive a 1099-MISC. I had to call the IRS last week for the same issue. I have reached out to sponsor and finally received one. The IRS rep did tell me that many companies do not report the W-9 until May or June as was the case with mine. The sponsor you won from may have reported your win. Might be worth calling the IRS. Also, the rep said that if I did not receive my 1099-MISC by next month that I could apply for an extension and the IRS could provide me with the information or i could 'estimate' and if it was too low or too high I would have to amend my taxes.
