Quantum Factory win?

I received an email this morning stating :
Congratulations! You've won our New Special Reward: My Secret Online Fitness Game Membership, Battles ZERO and ONE!

My Secret Online Fitness Game is a 16 week interactive spontaneous playfulness, that will make you exercise and take you out of your comfort zone - where actual change takes place.

They also attached an affidavit and wanted me to send them my picture to put on their website. I do not remember this sweepstakes and I looked it up and couldn't find anything. However , I did click on one of their links and it brought me to a blog but the blog post with the giveaway was dated yesterday.

I googled quantum factory and I found a page for South Africa, where I did live for a while. But then on the bottom of the email it gives a Hong Kong address.

I hate filling out an affidavit, especially when I am not feeling confident about the win.

I am thinking of maybe skipping this win. Has anyone else gotten an email like this or know more details about this win?

I won a fitness game I guess. It says its a 16 week interactive program. signed everything just waiting to see whats next
