Today in the mail I received a letter that said I won a 1,000 dollar shopping spree. It told me to fill out a w9 tax form and send it in. I've in the past received an affadavit when I've won. Has anybody else won something this big without an affy? The tax form needs my ss# and I'm not sure if I should send it in yet. I put a call into the name on the paper but it was to late in the evening. I guess I will have to wait and see what happens tomorrow. Wish me luck and hopefully a win.

I have had a couple that were just for cash that only required the W9. They just need that for the taxes. And yes, you do need to put your SS# on the W9.
 
My big wins have both required an affy but I think it's every sponsor's decision. They are required though to have you do the tax form which as the person before me said you DO need to include your SSN. Make sure it's a sweep you remember entering and that everything seems legit from what you can tell. We wouldn't want to see you get tricked!
 
Since value of $600, they require the tax forms, which also means you must claim at end of year/tax time.
