I need help!

Im not sure if i am posting in the right forum...I hope so. Summertime I won a paddle board from Hornitos. The ARV $1000. I signed, notarized and returned the document. I received that paddle board. When tax time started I emailed Hornitos to ask if they could tell me when I would receive the tax document. They emailed back and said they contacted the sponsor and the document would be sent out the first week in February. I never got it. Now March is more than half way over and still no document. I have emailed several times since and i always get a standard response that they have will contact the sponsor. I have since emailed asking for a name and phone number so I could talk to someone in person and no response. I emailed again and asked if i could have email or any info for the sponsor so i could contact them directly and no response. The cut off for filing taxes is coming fast and I cannot finish filing with out it. Can anyone please tell me what I need to do?
 
In my own personal circumstances, I have a prize I never got a tax document for and plan to list the ARV as "Other Income" (including Schedule 1) on a standard 1040 (I don't believe the 1040EZ forms have that section).
In your personal circumstances, you may or may not wish to speak to a tax advisor.

I think if you can find the same brand paddle board somewhere else for cheaper, print it out for your records and you can claim that amount instead of the $1,000 value.
Agree. Sweepstakes "values" tend to be inflated (especially trips). I would speak to a tax expert, but if the amount is smaller, it might not be an issue.

I think this is a pretty good idea if you don't receive the tax form. Keep for your own records in case there are any questions, but most likely you won't run into any issues.
