We have attached the Official Rules for complete details. To begin the prize claim process, we ask that you please do the following;
1. Read through the Official Sweepstakes Rules.
2. Assuming you do comply with the terms and conditions as stated in the rules, please complete:
a. The Affidavit of Eligibility and Liability/Publicity Release (attached)
b. The W-9 form (attached)
3. To complete the verification process, return both of the completed forms to us…along with a photocopy of your driver’s license (or any form of identification showing name and date of birth; all other information can be blacked out). These documents can be returned by fax (xxxx-xxxx-xx) or scan everything and email to [email protected].
PLEASE NOTE: The Official Rules allow each potential winner 5 days to complete the above prize claim steps. If you wish to accept the prize, we will need your completed paperwork by no later than Wednesday, December 5, 2018, or the prize will be forfeited and an alternate winner will be selected.
