Affidavit received with prize??

Received a package from 3M with safety glasses, painters paint, etc the other day. It also included an affidavit in it that requests it be notarized and returned. I am thinking that the affidavit was included more as a packing slip and not a requirement, they have already released the prize. This was from the 3M Safety First Sweepstakes and had 1000 of these prizes so I know other SA members have received the same thing. I have had more valuable prizes arrive from 3M unannounced and not have to do any paperwork.
 
Which sweeps? Is it possible you won a weekly/monthly prize and *also* the GP?

It was the 3M Safety First Sweepstakes. There is no grand prize, just 1000 of the prize package I received.

Yeah, sounds like a paperwork mix up, however, they might need the info to report to the IRS, since they'd probably want to write off $54,350.00

I got this too - it's a pretty cool prize.
I'm certainly not going to share my SSN or pay for notary for a $54 prize. I was wondering if I should send back the paperwork without those, though. Usually I ask when given an affidavit if they really need SSN/notarization - and for small prizes they almost always so "no".
I called her and left a message, so I'll let you know if she calls back.
