just got this email:
Hi Steve,
Thanks so much for your quick response. In order to be eligible to win, you must be at least 18 years or older, must be a legal resident of the US and cannot be an employee of Acer or the LPGA.
If you meet all the above eligibility requirements, to claim your prize, we will need you to fill out and sign the attached affidavit of eligibility and release form and fill out and sign the attached W9 form and return them to me no later than Wednesday, August 1st by 3pm PT or risk forfeiting your prize. Because the prize is valued at $699.99 you will receive a 1099 in that amount to file with your 2018 taxes. You can either fax them to the number below or scan and email as an attachment.
