Does any one know the up to amount for filing winnings in Alabama?

I only found out I won on Monday, I got my prize today, an xbox 360 with kinect. I also received my Loreal mascara today. This win came at a good time. Before I found out I won, I was thinking about giving up on sweepstakes.

I recently won an xbox 360 with kinect. The sponsor said it was valued at 400. I didn't get a 1099 form from them. I know that they reported my win to the IRS. I have never won anything I've had to pay taxes on before. I was hoping some one could tell me how to file this.

I received an email from deal ticker saying that I have won an x box 360, they want me to fill out an affidavit with my name address, and ssn. and fax this form to them. What should I do? By the way this sweepstakes was on face book.
